Marriage Forms for Pastors

Each of the forms below are now available with editable fields for data entry.

To use this new feature, you must first download each document to your own computer. Then you may open each form, enter the proper information and then choose "Save as..." to save a copy to your computer, or "Print" to print out the form with the typed entries. Do not use the "Submit" button on the forms; they must be printed and mailed as usual.
