Please complete the following to submit an application to seek a Declaration of Nullity:
- Complete and submit a Basic Data Sheet at the below link for each time you have been married, not including your present union. You can also download and print the application to submit to a Field Associate
- Obtain a copy of the civil marriage record for each marriage
- Obtain a copy of the filed and signed civil Decree of Dissolution for each marriage
Once submitted, a Field Associate or a member of the Tribunal will review your Basic Data Sheet and be in contact about the next steps of the process. We will request a copy of the civil marriage record and the civil Decree of Dissolution as the process moves forward. These documents can often take some time to obtain. It can be helpful to work towards obtaining copies of these documents while the Basic Data Sheet is being reviewed.
